We Value Your Feedback:  DG58 Annual Surveys

We invite all District 58 parents/guardians to take part in District 58’s annual surveys. Your feedback helps us to maintain a nurturing, child-centered learning environment for every student and guides the work of principals, teachers, and district administrators as they plan school improvement efforts. All responses are confidential and will not be tied to your student(s).

Parent/Guardian Survey Information:

We encourage our parents and guardians to take the District 58’s Parent/Guardian School Environment Survey, which takes just a few minutes and provides valuable feedback to District 58 to improve your child’s school experience. Paper copies are available at your child’s school. Please complete a separate survey for each District 58 school your family attends.

We also invite you to take the Illinois State Board of Education’s 5Essentials Parent Survey, which gathers parent feedback on key factors that impact student outcomes, like attendance and test scores. Your responses help guide school improvement. If at least 20% of parents participate, District 58 will receive data for that school. Paper copies are available at your child’s school. The survey is available in several languages, which can be selected on the login page. Please complete a separate survey for each District 58 school your family attends. 

 

Student Survey (Grades 4-8 only)

Students in fourth through eighth grade will be asked to participate in the 5Essentials Student Survey at school, between Tuesday, Feb. 10 and Friday, March 13. If you do not want your child(ren) to participate in this survey, please complete this Opt-Out Form (English / Spanish) and return it to your child’s school office by Friday, Feb. 6.

The Illinois 5Essentials Survey, developed by ISBE, helps schools improve and will be included in the 2026 Illinois State Report Card for your child’s school. Student, staff, and parent survey responses are confidential and not linked to individuals. Visit https://impactsurveys.my.site.com/s/article/survey-confidentiality to see the survey privacy policy. Fourth through eighth grade students can view the survey questions at https://impactsurveys.my.site.com/s/article/illinois-5essentials-survey-questions. The survey results will be used by your child’s school to inform planning.
