Please complete opt-out form if you wish to unenroll your child

Dear District 58 Families,

Our COVID-19 testing partner, Northshore, notified us that they will no longer be using the cheek swab method of specimen collection to ensure accurate testing in response to the current COVID-19 variant. Starting Monday, Jan. 24, they will exclusively use the shallow nasal swab to collect samples for all onsite COVID-19 testing.

If your child(ren) currently participates in the onsite COVID-19 testing program and you wish to withdraw permission for your child(ren) due to this or any other reason, please complete this testing opt-out form by Monday, Jan. 24. Form responses will be shared directly with your building nurse team and Northshore, and they will remove your child(ren) from the testing schedule. Please fill out one form per child.

If your child(ren) currently participates in onsite COVID-19 testing using the cheek swab method, and if you do not complete the opt-out form, your child(ren) will automatically transition to the nasal swab test.
