Dear District 58 Community,
Last month, District 58 partnered with Kodocare Pharmacy, District 99 and District 99’s other feeder districts to offer a two-session community vaccination clinic. As a reminder, the second vaccination clinic will take place this Sunday, and there are still appointments available.

This is an optional clinic open to any community member; at this time, there is no COVID-19 vaccine mandate for children.
The clinic will occur Sunday, Dec. 12 from 8 a.m. to 2 p.m. at Downers Grove North High School’s Learning Commons, 4436 Main St. Please use the Main Street entrance.
The following vaccinations will be available:
- COVID-19 first dose: ages 5-11
- COVID-19 first dose: ages 12 & up
- COVID-19 second dose: ages 5-11
- COVID-19 second dose: ages 12 & up
- COVID-19 boosters & third dose
- Flu: you can get both the COVID and flu vaccines in one visit
If you receive your first COVID-19 vaccine dose at Sunday’s clinic, you will need to schedule your second dose on or after Jan. 2, 2022 at a different pharmacy or clinic.
Vaccinations are by appointment only on a first-come, first-serve basis. You may register yourself and your family members for a vaccine. Community members will not be denied a vaccination for lack of insurance.

- Insurance card
- Vaccination Record card (Lost your vaccine card?)
