Kindergarten Requirement

State law requires that all students attending kindergarten, second and sixth grades undergo an oral health examination performed by a licensed dentist, and present proof to the school on a standard form by May 15 of that school year. District 58 encourages a yearly dental examination.

The law also requires that all students entering kindergarten (or the first grade attended) or sixth grade, as well as all students enrolling in an Illinois school for the first time, undergo a health examination, diabetes screening, and required immunizations.

In addition, parents/guardians of children between the ages of 6 months and 6 years must provide a statement from a physician that their child was "risk-assessed" or screened for lead poisoning in accordance with state law.

Below are standard dental and physical examination forms that you can print out and take with you to your appointment.
